Enrollment is easy and may be handled in one of three ways:  online, phone, or mail. Each of these options requires that you complete a simple form and submit it with payment for the annual (non-refundable) enrollment fee. Payment will be accepted in the form of a credit card, or if you are applying by mail, you may choose to pay with a personal check. Note:  You can enroll as a family or as an individual.

To join this program and begin realizing savings on your prescription drug costs, you pay an annual (non-refundable) enrollment fee as follows:
     > $25 per person ($20 if enrolling online)
     > $75 per family of four or more ($70 if enrolling online)

IMPORTANT: Before enrolling, please contact Customer Care at 1-866-809-9382 to estimate your cost savings.  The Enrollment Fee is non-refundable.
